Keep a food and exercise record for 2 - 5 week days & 2 weekend days noting time & amounts of food & fluids (except water) are consumed
EXAMPLE: 1 cup natural muesli with ½ cup full cream milk

Note down your medical history & relevant family medical history  eg father with type 2 diabetes
Bring:
    1. Recent blood test results (if you have them)
    2. A list of likes / dislikes
    3. A training or exercise diary if applicable,

Note: 
  1. Athletes  may prefer to bring their monthly / weekly training program (see below)​
    1. ​​Extra Form for Athletes or Regular Exercisers​
  2. No referral needed
  3. Health fund rebates with “extras cover” for dietitian (check with your fund to see if you are covered)
  4. Medicare registered for Team Care Plans
